Job Openings - New York, The Bronx

Support Services Community Outreach Worker $40,000 per year

 

This person must possess strong written and verbal communication skills; with the ability to problem solve while showing compassion and have crisis management capabilities. The ideal candidate for this position must be culturally sensitive, outgoing, and #LGBTQIA and HIV/AIDS friendly and committed to working with clients from various backgrounds including those with serious illnesses, addictions, disabilities, and varying socio-economic levels. 4-5 years of equivalent work experience with high-risk adults.


Previous training in homeless related social service systems preferred.


Duties and Responsibilities

  • Provide direct community outreach and health educational programs to targeted populations.
  • Participate in health fairs, neighborhood community activities, screenings, and special events.
  • Refer individuals to Destination Tomorrow for services. Set up health and wellness appointments as well as Destination Tomorrow appointments and provide reminder phone calls prior to every appointment.
  • Support in establishing a HIV services resource inventory of Medical and Mental Health organizations in the neighborhood as well as all 5 boroughs.
  • Support in completing organization intakes and making appointments for new clients referred for services at organization program intakes.
  • Support in identifying health and social priorities for the communities DT serves, including a review of community-level data on health disparities and identifying priorities using community-based knowledge and intelligence.
  • Provide data entry support into the organization’s data entry system as well as AIRS and eSHARE contractual data entry systems.
  • Assist in the planning and coordination of community health fairs and events with support services staff and outreach and prevention.
  • Support case management team clients by escorting to appointments and following up on service needs.
  • Work with patients, providers, and Destination Tomorrow care team to set goals for patient’s care.
  • Make home visits when needed to re-engage clients into care.


Minimum Requirements


  • Assist in the development and implementation of a marketing campaign to the community served by Destination Tomorrow.
  • Provide patient peer counseling on specific topics as directed by case manager and the Director of Support Services.
  • Maintain comprehensive documentation for data collection and provide statistical data and/or reports per grant requirements.
  • ABLE TO WORK FLEXIBLE HOURS, including some weekend availability.
  • Demonstrates a clear understanding of Destination Tomorrow mission, purpose, and philosophy.
  • Maintains confidentiality of all agency and patient related information. Protects patient rights as defined in the patient Bill of Rights.

Community Outreach Workers – (1 full-time & 2 part-time positions available) $19 hour

The organization emphasizes economic, social, and mental empowerment through a variety of holistic educational, financial, support-based, housing, and health programs.


The Outreach worker is the entry level of work providing health and disease prevention information to medically underserved populations in the LGBTQI + community, assisting these individuals in adopting healthy behaviors.


Knowledge/Experience working with HIV/AIDS, Covid-19 and Monkey Pox is a big plus!

Youth Services Coordinator – $45,000 per year depending on experience

The Youth Services Coordinator must possess innovative ideas and strategies to engage and retain high-risk youth. The Youth Services Coordinator will be responsible for outreach, promoting youth drop-in events and programming, as well as group facilitation for high-risk young people ages 13-24. 


The ideal candidate for this position must be motivating, and inspiring with an understanding of building positive relationships with vulnerable young adults while maintaining professional boundaries. 


The Youth Services Coordinator must have effective communication skills, knowledge, willingness to form collaborative relationships with other organizations, effective listening skills, the ability to objectively think and problem solve with the aptitude to work alone or as part of a team.


Primary Duties and Responsibilities:


  • Work closely with the Director of Programs to develop programming, youth events, social media campaigns, and ads to engage young adults from ages 13-24
  • Identify and conduct presentations to schools, other agencies, and community groups about the programs and services offered at Destination Tomorrow
  • Participate as an active and contributing member of the KiKi Coalition
  • Build and maintain relationships with local colleges and high schools to assist with implementing GSAs (Gender & Sexuality Alliances); as well as develop and nurture relationships with established GSAs
  • Organize and run the youth services drop-in space (DaSpotNextDoor) and KiKi Lounge
  • Ensure DaSpotNextDoor space is managed correctly by making sure youth using the space are following rules and safety practices; making sure metro cards and food are provided, as well as activities are held to engage young people
  • Act as lead contact and liaison for all programs and youth-based services


Minimum Requirements:


  • 3+ years of working with young people
  • Understanding the youth population and positive youth engagement and relationships
  • Ability to effectively minimize danger in high-risk youth
  • Proficient in using computer software, the internet, and email
  • Excellent verbal and written skills with exceptional attention to detail
  • Personal qualities of integrity, credibility, and a commitment and passion for Destination Tomorrow’s mission

Support Services Case Manager $50,000 per year

Destination Tomorrow believes that it is more helpful to empower our most vulnerable LGBTQ+ community members in a way that takes them off the path of needing emergency care. The organization emphasizes economic, social, and mental empowerment through a variety of holistic educational, financial, support-based, housing, and health programs.


The new Case Manager must possess strong written and verbal communication skills; with the ability to problem solve while showing compassion and have crisis management capabilities. The Case Manager will be working under the direction of the Director of Support Services, conducting intakes, providing service plans and comprehensive case management services to members within the LGBTQ, individuals with HIV/AIDS diagnosis, homeless, and underprivileged communities. The ideal candidate for this position must be culturally sensitive and committed to working with clients from various backgrounds including those with serious illnesses, addictions, disabilities, and varying socio-economic levels. In addition, the candidate must be organized, have conflict resolution skills, and have a good understanding of government programs, support networks, community resources and working with a team.


Primary Duties and Responsibilities:


  • Conduct face to face needs-based assessment and determine the best course of action to assist clients
  • Create and monitor service plans, consistently review case progress and determine case closure or elevation to the Director of Support Services
  • Assist clients to make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Arrange appointments with social service, health, and governmental agencies
  • Record case information accurately and within a timely manner in designated computer software
  • Participate in case management, CQI, case presentation, general staff, Part D specific and supervisory meetings
  • Maintain documentation of case management activities and case management charts including referral services, and outreach activities.
  • Adolescent supportive services recruitment, education/counseling, facilitating groups, community escorts to increase referral success/advocacy, seek out and empower HIV+ and homeless clients who are at risk of or fallen out of care.
  • Perform other related duties as assigned by Director of Support Services.
  • Home visits when necessary to re-engage clients that have been loss of contact.


Minimum Requirements:

Bachelor’s degree or 4-5 years equivalent work experience with high-risk adolescents.


Previous training in HIV/AIDS, homeless related social service systems preferred.

Experience working with LGBTQIA+ and sexually exploited community.

Community Health Worker $48,000 per year - Job ends 5/31/2023

The COVID-19 Community Health Outreach Worker is the entry level of work providing health and disease prevention information to medically underserved populations in the LGB and TGNCGNB community, assisting these individuals in adopting healthy behaviors. Employees in this classification do not supervise.


Duties and Responsibilities

  • Provide direct community outreach and health educational programs to targeted populations.
  • Participate in health fairs, neighborhood community activities, screenings, and special events.
  • Refer individuals to Destination Tomorrow for services.
  • Conduct cultural, LGBTQ sensitivity trainings
  • Support in identifying health and social priorities for the communities DT serves, including a review of community-level data on health disparities and identifying priorities using community-based knowledge and intelligence.
  • Support in establishing a COVID-19 resource inventory of vaccination and testing sites in the neighborhood.
  • Support in developing a partner engagement plan for the Mott Haven neighborhood that outlines the neighborhood coalition-building activities and direct community services.
  • Assist in the planning and coordination of community health fairs and events.
  • Assist in the development and implementation of a marketing campaign to the community served by Destination Tomorrow.
  • Provide patient peer counseling on specific topics as directed by social worker, case management, and the Director of Outreach.
  • Maintain comprehensive documentation for data collection and provide statistical data and/or reports per grant requirements.
  • Demonstrates a clear understanding of Destination Tomorrow mission, purpose, and philosophy.
  • Maintains confidentiality of all agency and patient related information. Protects patient rights as defined in the patient Bill of Rights.
